Recurring Billing Policy

Our subscription model is a monthly recurring charge and the billing period begins from the time of registration, as defined by when you enter your payment method. By agreeing to our recurring billing terms, you (“the patient”), are agreeing to the following:

The patient represents and warrants that (i) any credit / debit card information the patient supplies is true, correct and complete, (ii) charges incurred by the patient will be honored by the patient’s credit/debit card company, (iii) the patient will pay the charges incurred in the amounts posted, including any applicable taxes, and (iv) the patient is the person in whose name the credit / debit card was issued and the patient is authorized to make a purchase or other transaction with the relevant credit / debit card and information.

The patient agrees and authorizes the payment method to be billed automatically for the entire subscription length, according to the published pricing on the Cerebral Inc. website, which is subject to change at any time.

The patient agrees and authorizes the payment method to be billed beginning on the date of registration and subsequently on a monthly basis regardless of whether the assessment has been completed or services have been rendered by (including, but not limited to, prescribing provider visits, Care Counselor visits, and medication prescription and delivery).

If Cerebral Inc. is unable to secure funds from the patient’s debit / credit card(s) for any reason, including, but not limited to, insufficient funds in the debit / credit card or insufficient or inaccurate information provided by the patient when submitting electronic payment, Cerebral Inc. may undertake further collection action, including application of fees to the extent permitted by law.

The patient has the right to revoke this authorization by contacting Cerebral Inc. via email by 9am PT one business day prior to the scheduled payment date and following the cancellation process until one of our Coordinators confirms the cancellation. The patient understands and acknowledges that services may be cancelled or withheld if the patient revokes this authorization, and that patient is still responsible for all charges incurred by the patient or otherwise owed to Cerebral Inc. This authorization will remain in full force and effect until revoked by the patient or Cerebral Inc.

Our Care Team at support@getcerebral.com is always available to resolve any concerns or confusion regarding payment. The patient acknowledges and agrees he or she will not dispute the payment with the credit / debit card company, provided the transactions correspond to the terms indicated in this authorization form.

All orders are final once shipped. When the package is dispensed by the pharmacy, the order cannot be returned or refunded. Federal law prohibits pharmacies from accepting returns or exchanges on prescriptions, to protect the integrity of the medication and the health of the patient. If you think you’ve received the wrong medication, have received an order in error, or have any other questions, please contact us at support@getcerebral.com.
